Flow Path provides you with a overview of how patients are progressing in a care flow using a Sankey-style visualization. Use Flow Path to visually identify where patients are engaging or dropping off and track the volume of patients in each track and step. Think of it as a bird's-eye view of patient flow with the ability to zoom in wherever you want.
Accessing Flow Path
Navigate to Improve > Flow Path
Select a care flow you want to open in Flow Path
Once your care flow is opened in Flow Path you can:
Select a version - the latest published version is shown by default but you may select other versions if needed
Set a date range: Use the time filter to set the date range for the data shown in the Flow Path or select from predefined options like Last 7 Days, Last 30 Days (default).
Understanding the Flow Path Visualization
Flow Path helps you see how patients move through a care flow by representing it as a series of connected elements(tracks and steps) and flows (patients transitions).
Elements:
These represent different parts of the care flow:
Tracks: These are the tracks that you have built within your care flow. Each track is represented by a tall turquoise rectangle.
Steps: Individual actions or checkpoints within a track. These appear when you click to expand a track and are represented by a tall green rectangle.
The red part of each element indicates ‘Non-completed percentage’, offering quick insights into potential bottlenecks.
Flows:
Flows connect elements (tracks and steps), showing the direction of patient movement. Think of them as hallways guiding patients from one part of the care flow to another.
They provide a visual indicator of how patients transition between different elements.
Flows cannot be expanded but can be repositioned on the canvas if necessary (by holding and dragging)
How to Navigate:
Expand Tracks: Click on a track to reveal its steps.
Expand Steps: Click on a step to view the Action card, which lists all actions in that step along with completion times and rates.
Hover for details: Hover over a track or step to see additional information like completion rates or average completion time.
Zoom and Drag: Use the zoom controls to zoom in and out, or click and drag elements to adjust your view.
Hide/Show Elements: Right-click on any element (track or step) to hide it from view. You can also toggle between Show All or Hide All Elements (then clicking only on the elements you want to see).
Custom Views
Once you have adjusted a Flow Path to your needs you can save it under 'My saved views' allowing you preserve your configuration (expanded tracks, excluded elements, zoom level, and layout). Saved Flow Path views will always be updated with the latest latest data.
Note: Saved flow Paths are stored on your account and can't be shared or accessed by other users in your organization at this time.
Data
Flow Path is powered by orchestrated care flow data, offering up-to-date insights from your ongoing care processes.
Data Availability: Data is accessible starting from January 2024.
Data Refresh Frequency:
In Sandbox, data is refreshed every hour.
In Production, data is refreshed every 15 minutes.
Comprehensive Data Points: The visualization includes key metrics such as:
Patient Counts: Total number of patients at each element (track or step).
Completion Rates: Includes started and completed counts for every element, helping you track patient progress.
Completion Times:
Median Completion Time: Provides a general view of patient flow, highlighting the typical experience.
Average Completion Time: Offers a complete picture, factoring in both typical cases and outliers (e.g., unusually fast or slow patients).
