Define your form settings or perform specific (bulk) form actions

Creating your first form with question & answer options is straightforward, though sometimes you need more advanced functionality to easily adopt Awell forms in your organization.

This article outlines how to set a Custom form key for coding purposes, Add a Trademark to a validated questionnaire or perform Bulk operations to ease the configuration work.

Access Form settings

You can access Form settings by navigating to your Form from either the "My Form Library" section on the Studio home page, or by selecting a specific Form action in a care flow.

Set a custom form key

By default, the name of your form is set as custom form key. You have the option to alter the custom form key and enter certain coding that makes sense for your use case.

Coding can be used to standardize clinical terms and give more meaning to the data you are collecting, making sure the collected data adheres to a specific registry, standard outcomes set (like ICHOM), or research protocol. Coding also helps to make data analysis easier.

πŸ‘€ tip: the form key is appended to the data point and thus visible when setting conditions and logic in your care flow. Therefore keep the custom form key short & sweet, so it's easier to use in practice when building.

Set a trademark

For certain proprietary or validated forms, you may need to adhere to certain vendor requirements such as identifying the source of the Form. Therefore you can set a trademark for your Form, which will be visualized towards your stakeholders when they fill out the form in either Hosted Pages or Awell Care.

Perform bulk operations

To speed up the manual configuration work within a Form, you can mark all questions of a Form as required or optional, by performing a bulk operation on your From through the Settings:

Export form

You can export a Form with all questions and answers to then import into a new (empty) Form. You may want to do this if you're looking for an almost identical copy of an existing form or when needing to use the Form outside of the Awell Platform.
