Adding forms to steps allows you to organise data collection in the care pathway. You can reuse existing forms such as validated questionnaires and PROMs, PREMs or forms other team members made. You can also create a custom form from scratch.

How to add a form to your care pathway?

  1. Double click on the step where you want to collect data

  2. Press "Add action" and select "Form" in the drop-down list.

You now have different options:

  • Select an existing form from the dropdown or create a new one by choosing "Add form"

  • Define who should fill in the form

  • Define where this form should be filled in
